Undergraduate Research Approval Form

Use this form if you have registered for any 499 Credit and would like to request that the credits be used in your biology degree.

Biology Petition Form

Use this form if there is a course NOT listed on your degree handout that you would like the department to consider for credit in your degree requirements.
